1. Fill Your Name
2. Fill Your Country of Residence
3. Tick Individual in this Section

Part 2 - Tick 9(a) and Enter your Country of Residence.

Part 3 – Tick 11. It Must be the Only option.

Part 4 – Print the Filled Form and Sign It and Write the Date.

Now You Need to Fax it or Scan it in order to send to the advertising company. You need not fill anything else here.
